  • Eligible Individual living on Native American Tribal Lands
  • State Supplement to the Aged, Blind, or Disabled
  • Aid to Families with Dependent Children

If you qualify for Assurance Wireless under this method, you do not have to provide supporting documentation, but sign under oath of perjury. If you qualify under this method and the next, it is best to apply using this method, because the application is easier.

The last two requirements for qualifying for a Assurance Wireless free government cell phone in Connecticut is that only one Lifeline Assistance phone is allowed per household.
